{"id":3746,"date":"2026-07-12T16:36:08","date_gmt":"2026-07-12T09:36:08","guid":{"rendered":"https:\/\/moderncancerhospital.net\/?page_id=3746"},"modified":"2026-07-27T05:06:56","modified_gmt":"2026-07-26T22:06:56","slug":"photodynamic-therapy","status":"publish","type":"page","link":"https:\/\/moderncancerhospital.net\/id\/technology\/photodynamic-therapy\/","title":{"rendered":"Terapi Fotodinamik"},"content":{"rendered":"<h1>Terapi Fotodinamik (Photodynamic Therapy\/PDT)<\/h1>\n<h2>Apa Itu Terapi Fotodinamik?<\/h2>\n<p>Terapi fotodinamik, yang sering disingkat sebagai PDT (Photodynamic Therapy), merupakan metode pengobatan kanker yang mengombinasikan obat peka cahaya (photosensitizer) dengan cahaya menggunakan panjang gelombang tertentu.\n\nKetika senyawa fotosensitizer terpapar oleh cahaya dengan panjang gelombang yang sesuai, senyawa tersebut akan memicu terbentuknya oksigen reaktif yang mampu menghancurkan sel-sel di sekitarnya, termasuk sel kanker.\n\nTerapi ini telah mendapatkan persetujuan klinis dari Badan Pengawas Obat dan Makanan Amerika Serikat (U.S. Food and Drug Administration\/FDA) pada tahun 1996. Sementara itu, Badan Pengawas Obat dan Makanan Tiongkok (China\u2019s State Food and Drug Administration) memberikan persetujuan klinis untuk metode ini pada tahun 2003.<\/p>\n<h2>Bagaimana Cara Kerja PDT dalam Pengobatan Kanker?<\/h2>\n<p>Proses pengobatan PDT (Photodynamic Therapy\/Terapi Fotodinamik) diawali dengan pemberian zat fotosensitizer melalui suntikan intravena ke dalam aliran darah pasien. Meskipun zat ini akan beredar ke seluruh tubuh dan diserap oleh berbagai sel, zat fotosensitizer cenderung bertahan lebih lama di dalam sel kanker dibandingkan jaringan sehat.\n\nSekitar 24 hingga 72 jam setelah penyuntikan, ketika sebagian besar zat tersebut telah dibersihkan dari sel normal namun masih terkonsentrasi di dalam jaringan tumor, area kanker akan dipaparkan dengan cahaya khusus. Zat fotosensitizer yang tertahan di dalam tumor akan menyerap cahaya tersebut dan menghasilkan spesies oksigen reaktif (reactive oxygen species\/ROS).\n\nZat reaktif ini kemudian menyebabkan kerusakan pada sel kanker di sekitarnya, sehingga membantu menghancurkan dan membunuh sel-sel kanker secara terarah.<\/p>\n<p>Selain efek sitotoksik langsung terhadap sel kanker, PDT (Photodynamic Therapy\/Terapi Fotodinamik) juga dapat mengecilkan atau menghilangkan tumor melalui dua mekanisme tambahan:\n\nPertama, PDT dapat merusak pembuluh darah yang memasok nutrisi ke tumor, sehingga aliran darah dan suplai nutrisi yang dibutuhkan kanker untuk berkembang menjadi terhambat.\n\nKedua, terapi ini dapat merangsang respons sistem kekebalan tubuh, sehingga tubuh terdorong untuk mengenali dan menyerang sel-sel tumor secara langsung.<\/p>\n<p>Sumber cahaya yang digunakan dalam PDT (Photodynamic Therapy\/Terapi Fotodinamik) dapat berupa laser atau jenis pencahayaan lainnya. Apabila menggunakan laser, cahaya dapat dialirkan melalui kabel serat optik (fiber optic)\u2014yaitu serat tipis yang mampu menghantarkan cahaya\u2014untuk mencapai tumor yang berada jauh di dalam tubuh.\n\nSebagai contoh, kabel serat optik tersebut dapat dimasukkan melalui endoskop, yaitu alat tipis berlampu yang digunakan untuk melihat jaringan di dalam tubuh, guna menghantarkan terapi langsung ke area kanker pada organ seperti paru-paru atau kerongkongan (esofagus).\n\nUntuk tumor yang berada di permukaan tubuh atau dekat dengan permukaan kulit, seperti kanker kulit, sumber cahaya eksternal seperti light-emitting diode (LED) atau jenis pencahayaan lainnya dapat digunakan sebagai pengganti.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-3747\" src=\"https:\/\/moderncancerhospital.net\/wp-content\/uploads\/2026\/07\/Photodynamic-Therapy-001.webp\" alt=\"Photodynamic Therapy\" width=\"620\" height=\"300\" srcset=\"https:\/\/moderncancerhospital.net\/wp-content\/uploads\/2026\/07\/Photodynamic-Therapy-001.webp 620w, https:\/\/moderncancerhospital.net\/wp-content\/uploads\/2026\/07\/Photodynamic-Therapy-001-300x145.webp 300w, https:\/\/moderncancerhospital.net\/wp-content\/uploads\/2026\/07\/Photodynamic-Therapy-001-18x9.webp 18w\" sizes=\"auto, (max-width: 620px) 100vw, 620px\" \/><\/p>\n<h2>Keunggulan PDT (Photodynamic Therapy\/Terapi Fotodinamik)<\/h2>\n<ul>\n<li><strong>Minim invasif:<\/strong> Prosedur ini tidak memerlukan sayatan pembedahan, sehingga mengurangi rasa sakit dan ketidaknyamanan yang dirasakan pasien.<\/li>\n<li><strong>Respons terapi yang cepat:<\/strong> Efek pengobatan umumnya dapat mulai terlihat dalam waktu 48 hingga 72 jam setelah terapi dilakukan.<\/li>\n<li><strong>Mengurangi risiko kekambuhan:<\/strong> Terapi ini mampu menghancurkan lesi kanker kecil yang tersembunyi, sehingga membantu menurunkan kemungkinan tumor kembali berkembang (rekurensi).<\/li>\n<li><strong>Tingkat komplikasi rendah:<\/strong> Karena memiliki toksisitas yang rendah, PDT sangat sesuai untuk pasien dengan kanker stadium lanjut, pasien lanjut usia, maupun pasien dengan kondisi fisik yang kurang baik.<\/li>\n<\/ul>\n<h2>Potensi Komplikasi dan Efek Samping<\/h2>\n<p>Salah satu efek samping yang perlu diperhatikan berkaitan dengan penggunaan obat fotosensitizer porfimer sodium, yang dapat menyebabkan kulit dan mata menjadi lebih sensitif terhadap cahaya selama kurang lebih enam minggu setelah terapi.\n\nOleh karena itu, pasien disarankan untuk menghindari paparan langsung sinar matahari serta cahaya dalam ruangan yang terlalu terang selama masa pemulihan tersebut.<\/p>\n<p>Karena zat fotosensitizer cenderung terakumulasi lebih banyak pada jaringan tumor dan cahaya pengaktif difokuskan langsung pada area tumor, kerusakan pada jaringan sehat di sekitarnya umumnya dapat diminimalkan.\n\nNamun, pada beberapa kasus, PDT (Photodynamic Therapy\/Terapi Fotodinamik) tetap dapat menyebabkan efek samping pada jaringan sehat di sekitar area terapi, seperti luka bakar, pembengkakan, rasa nyeri, atau terbentuknya jaringan parut (bekas luka).<\/p>\n<p>Efek samping tambahan umumnya berkaitan dengan lokasi tubuh yang menjalani terapi. Beberapa efek yang mungkin muncul meliputi batuk, kesulitan menelan, nyeri perut, nyeri saat bernapas, atau sesak napas.\n\nGejala-gejala tersebut biasanya bersifat sementara dan akan membaik seiring waktu setelah pengobatan.<\/p>\n<h2>Jenis Kanker yang Saat Ini Dapat Ditangani dengan PDT (Photodynamic Therapy\/Terapi Fotodinamik)<\/h2>\n<ol>\n<li><strong>Kanker orofaring:<\/strong> Tingkat efektivitas PDT dapat mencapai hingga 75% pada kanker mulut stadium awal dan kanker nasofaring stadium awal.<\/li>\n<li><strong>Kanker esofagus (kerongkongan)<\/strong> PDT dapat secara signifikan membantu mengatasi sumbatan yang disebabkan oleh kanker esofagus yang berkembang.
